Corrections are the most useful thing to send. If a rate, bracket or deduction on a page disagrees with the state or federal source it cites, say which page and which figure, and link the source if you have it. The figure will be checked against the source and, if the site is wrong, fixed with the page's verified date updated.

Questions about how a calculation was done are welcome; questions about what you should do about your own taxes are outside what this site can answer, and the reply will say so and point to the methodology page.

There is no form because a form would send what you type to a server, and this site does not do that.
